The practical NDIS audit-readiness checklist

Audit readiness is less about a last-minute document sprint and more about being able to show how your organisation works on an ordinary Tuesday.

1. Confirm your scope

Start with the registration groups and supports you actually deliver. Your policy set should reflect that scope, including any specialist or higher-risk modules.

If the scope has changed, record the decision and the date. A clear scope story helps an auditor understand why a document is present—or intentionally out of scope.

2. Make control visible

Every controlled policy needs an owner, version, review date and approval trail. Drafts should be visibly draft, and superseded copies should not be circulating in shared folders.

A register is useful only when it reflects the current working version. Pair document control with read receipts, review notes and a simple change history.

3. Connect policy to practice

Auditors look for evidence that staff know what the policy asks them to do. Training, competency sign-off, supervision and worker registers should connect back to the relevant policy.

Participant records should show choice, goals, risks and progress in language that is specific enough for another worker to understand.

4. Rehearse the evidence

Before the audit, sample a participant journey, a worker file and a high-priority risk. Follow each trail from policy to action to evidence and note where the hand-off breaks.

The goal is not to manufacture perfect files. It is to find the small gaps while there is still time to fix them.
